LFSC offers several different Membership Plans throughout the year.-

LFSC offers a payment plan for families whose membership fees total over $400. Installment payments are charged to yoru credit card on April 5th, May 5th and June 5th.

Registration Forms
Complete the Family Registration Form (Form A) and a Swimmer Registration Form for each swimmer. Swimmer Form B (Yellow/Green/Bronze) can can be used to register up to 5 swimmers. Swimmer Form C can be used to register up to 3 Sr swimmers.-
Each LFSC family must complete this form. Returning members can make changes to their contact information then skip to the the signature portion of the form.

New members should call Coach Laurel Liberty for an evaluation and placement. Returning members should re-register for their current squad or consult a coach for registration guidance.

Meet Schedules and Changes
Complete these forms for entry into meets or changes to your current meet schedule. Please do not use the Change form as your initial Schedule form.-
Complete this form to register for meets for the Long Course season. Be sure to sign-up for the entire season; you can make changes later using the Change form.

If your schedule changes you can make changes to a previously submitted Meet Schedule using this form. Your Change request can only be accommodated if the form is received prior to submitting the entry.
