- Late Entries -- Any team or athlete missing the entry deadline will be permitted to submit a Late Entry subject to certain restrictions. ALL ENTRIES CLOSED
- New Entries -- Entries for athletes achieving qualifying standards for the first time between March 1 and March 5, 2012, may be submitted subject to certain restrictions. ALL ENTRIES CLOSED
- Scratches -- Scratches may be made online subject to deadlines, rules and procedures in the Meet Information and other places.
- Positive Check-in -- You can check-in for the 800 free, 1500 free and all relays using this form. Remember to complete this form by established Check-in deadlines.

All USA Swimming members who are properly registered with the following LSCs are eligible to compete in this 2012 Spring Speedo Champions Series Meet: Illinois, Iowa, Minnesota, North Dakota, South Dakota and Wisconsin.
Up to 150 USA Swimming members who are properly registered with other LSCs will be permitted entry into the meet. Each non-resident swimmer must have achieved the qualifying standard in one or more individual events. Non-resident swimmers are eligible to swim Bonus Events and Time Trials.
Non-resident teams must complete a Team Reservation Form and submit a $25/swimmer deposit to secure entry. Reservations will be accepted on a first-come basis until the established limit has been met. Payment can be made by credit card through Google Checkout or by check (mail). If paying by check please mail the Team Reservation Form and check to :
